Start Using the Authorized Payment Service

To start using the Authorized Payment Service now, follow these instructions:

1. Download the application form (in PDF format, 57 KB)
2. Provide all the required information in block letters
   • Be sure to complete all sections of the form.
   • Check off your choice of payment option.

   See the example below to complete the sections for "Transit" and "Chequing account number".

   

3. Attach a copy of a personal cheque from your financial institution clearly marked VOID.
4. Write the fax number in the space provided on the form and send your application and copy of your check by fax to:
   • 1-888-875-0444

   • Mail your application to:
     VISA Centre, P.O. Box 3634 Station Terminal, Vancouver, BC V6B 3Y8

You can access the Application Form by using Adobe Reader, a program available free of charge on the Adobe Web site.
